Doctor,

More than 5yrs I am suffering with piles and fissure. I am suffering with constipation and its not regular. Stool becomes harder and its not coming out easily and I need to release it with lots of pain and blood stains. Two , three days continuously motion is not coming out and I am suffering with abdominal pain and bulged abdomen. Gas is also not releasing normally. Doctors suggested me to go for surgery for fissures but i dont want to. Whenever I release hard stool with blood I am crying with pain and after that I am getting severe back pain and it aggravates downwards till my toes. Please suggest me a medicine.

Thanks

Hello,

As you are suffering from chronic piles and fissures since more than 5 years, the primary reason could be sedentary life style and diet, which are incompatible. Another prominent reason could be genetics. Due to irregularity in digestion and metabolism, there are chances that you get constipated and you need to set a goal to follow a proper diet regimen aimed at relieving the constipation and thus allowing easy expulsion of the stool. The bleeding is caused by aberration of the hard bowel exerting on the intestinal walls. With proper diet and medication, it may be brought under control and maintained without further exacerbation in future. If the condition worsens in terms of bleeding or pain, you may need to consult nearby ayurvedic doctor for non-surgical interventions and therapies which are aimed to relive your symptoms as well as the disorders, the common procedure is done in Ayurveda is called as Kshara Karma and it can be followed if indicated.

In order to control relieve your constipation and abdominal bloating, and flatulence, you may need to follow certain medications which are helpful in your condition such as Arshakalp Vati, and Triphala Churna 1 tsp internally at bed time for relieving your constipation at initial stages and if constipation is severe, you may need to follow Nityam Churna.

Fissure in the anus usually heals by itself in 2weeks. Even if it persists, the proper diet or medicines that help to soften the stool and if maintained without constipation will help to heal. The tone of the muscles around anus must be relaxed and constipation tends to create spasms in the muscles which worsens the condition. When relaxed the flow of blood around the anus will be increased which helps to heal better.

Medicines must be taken to soften and regulate the bowel movements. You can take Triphala powder, Abaya aristam - 20ml after food twice daily, Agastya Rasayanam + Mahatiktaka ghrtam - 1tsp before food morning and night which will help with the fissure and constipation. You can take it for a month and then with the help of diet you can maintain it.

Apply Mahatiktaka ghrtam in the anus and try to have some warm tub bath for 10-20min.

Include complex carbohydrates, fiber, fruits and vegetables in the diet. Include raisins, yam, berries, apricots, flax seeds, papaya, pine apple, green leafy vegetables, prunes, figs that improve the bowel movements. Drink enough water. Do exercise or Yogasanas like pavanamuktasana, vajrasana, matyasana, bhujangasana stimulate the peristaltic movements and improve the bowel movements.

Hello,

As you are complaining severe and chronic constipation due which you are suffering from pain abdomen, and as you are having the history of piles and fissures, it is important to take care of the problem as soon as possible to avoid further anorectal complications. For this, you need to consume ayurvedic medicines and the goal of treatment starts to correct your constipation which is very important for which you need to be on fiber-rich food, and with balanced nutrition. You have to consume the food in such a way that the food should get digested easily. The initial medicine in ayurveda is Triphala Churna which has to be consumed at bed time with lukewarm water. There are other medicines to be followed such as Kutajavleh to be consumed with twice daily after food or one hour before food and dosage is 1tsp. For external application at anal region, you may apply the medicated oil called as Jatyadi Tel which is very effective and you can apply either before or after defecation which is helpful in healing fissures and piles masses and even internal piles also.

With regards, Dr.Vijay.
